Coastal Manor employee accused of elder abuse following incident with 90-year-old resident

An employee of the Coastal Manor nursing home in Ludowici is being charged with elder abuse following an incident with one of its residents.

According to a Ludowici Police Department incident report, an officer responded April 10 on the report of elder abuse from the day before. An employee told the officer that the suspect approached a 90-year-old resident to take her outside. The employee said that the suspect looked at the 90-year-old woman and said, “You put the hell in health care.”

The employee also said that the patient had been distraught prior to her encounter with the suspect and that after he spoke to her allegedly in an inappropriate manner, it upset her more.

The employee added that as the suspect was pushing the woman in her wheelchair across the threshold of the doorway, he allegedly used enough force to push the wheelchair forward and throw the patient out of the wheelchair and on to the ground, causing a skin tear on one of her legs.

The employee stated that they did not witness the incident but that another employee had observed the encounter and reported it to her supervisor, later making a statement to the police after the initial report had been made to authorities.

The suspect was arrested on April 11 and made bond on $5,000 bail.

The elderly patient also complained of pain to her arm and shoulder.

Manager and Employee of Assisted Living Facility Arrested for Exploitation and Theft Against an Elderly Person

TALLAHASSEE, Fla.—Following an investigation by Attorney General Ashley Moody’s Medicaid Fraud Control Unit, Tavetta Lavetta Jones and Tekera Levine were arrested over the weekend on charges related to abuse and exploitation of a senior. As employees of an assisted living facility in Escambia County, Jones and Levine were supposed to transfer the victim to sign bond paperwork. Instead, Jones and Levine, Jones's manager, abandoned the victim on the side of a road and took the victim’s identification card and debit card. Jones is charged with exploitation of an elderly person or disabled adult, and criminal use of personal identification information. Levine faces a charge of accessory after the fact.

Attorney General Ashley Moody said, “The scheme concocted and carried out by these two caregivers is horrifying—the women abused their positions to steal a patient’s ID and debit card, then abandoned the senior victim on the side of the road. Thankfully, following an investigation by my Medicaid Fraud Control Unit, these suspects have been arrested and will face serious charges for this criminal scheme.”

The MFCU investigation revealed that Jones, an employee of Whispering Pines Assisted Living Facility, abandoned an elderly resident on Antioch Cemetery Road in Walton County on Sept. 3, 2019. The elderly victim identified Jones and Levine as the facility employees who orchestrated the abandonment and identified a black Volkswagen as the vehicle Jones was driving while abandoning the victim. Jones and Levine left the victim without a phone, water, money or identification on a particularly hot day.

Levine, the manager of Whispering Pines, gave varying accounts about the occurrence of the abuse and Jones’s involvement. Phone records placed both Jones and Levine in Walton County near the time law enforcement responded to a 911 call by residents on Antioch Cemetery Road. Phone records also place Jones in the vicinity of ATMs where the victim’s debit card and personal identification number were used to gain access to the victim’s bank account during August and September 2019. Video footage places a black Volkswagen or similar vehicle at the ATMs during the transactions involving the victim’s debit card and PIN.

The Escambia County Sheriff’s Office arrested Jones on one count of exploitation of an elderly person, a third-degree felony; one count of criminal use of personal identification information of a person over 60 years of age and one count of criminal use of personal identification information over $5,000, both second-degree felonies. If convicted on all counts, Jones faces a possible sentence of up to 35 years in state prison.

ECSO arrested Levine on one count of accessory after the fact, a third-degree felony. If convicted, Levine faces a possible sentence of up to five years in state prison.

The cases will be prosecuted by Assistant Attorney General Christi Hankins of MFCU through an agreement with Ginger Bowden Madden, State Attorney for the First Judicial Circuit of Florida.

Las Vegas senior facility owner charged with abusing residents

The owner and an employee of a Las Vegas senior facility were arrested on elderly abuse charges this month.

Ravette Taylor and Richard Cisneros, who were arrested Jan. 20, are each charged with seven counts of abuse of an older or vulnerable person, according to court records.

A certified nurse’s assistant who previously worked at “The Ohana Independent Living Home” called police Jan. 13 to report two elderly women were being sexually abused, beaten and overmedicated at the senior care facility, according to an arrest report from the Metropolitan Police Department.

Police found eight residents, including a man suffering from Parkinson’s disease who was immobile, another man who was blind, deaf and suffering from dementia and six women suffering from advanced dementia. Taylor told police that neither she nor any employees had training in the medical field.

In her interview with police, Taylor, the owner, said she bought the home from a registered nurse in 2018.

She told police she didn’t get the property registered as a group home, which would allow her to care for patients with advanced needs, because the fire department told her that getting the home up to code would cost $16,000. Instead, she registered it as an independent living facility.

“She admitted she was aware the residents needed a high level of care and admitted her business was not providing the care required,” police said in the report.

“When asked she stated she had never presented herself as a medical professional to the social workers at the hospital but had never told the social workers that the prior owner was no longer associated with the business and that (Taylor) was not a medical professional,” the report continued.

Cisneros told police he was not familiar with caring for patients who are immobile and was unsure of how often to change their positions to prevent bedsores. He said he usually moved patients once at the beginning and end of his shift.

Police redacted the location of the home in the report, and the secretary of state’s business licensing department had no record of the facility.

Cisneros was released without bail, and Taylor was released after posting bail. The two are scheduled to appear in court again Feb. 22.

Las Vegas senior facility owner charged with abusing residents

Nursing home employee arrested, suspected of elderly abuse

On December 28th 2018, Senior Trooper J.A. Tupper arrested Adam Wheeler on three counts of Abuse and Neglect of Incapacitated Adults causing bodily injury, one count of Abuse and Neglect of Incapacitated Adults causing serious bodily injury, and four counts of crimes against the elderly.

WELCH — An employee of Golden Harvest Nursing Home, located in Welch Community Hospital, was arrested by West Virginia State Police in connection to abuse and neglect.

Senior Trooper Jarod Tupper arrested CNA Adam Wheeler on Dec. 28 on, "Three counts of Abuse and Neglect of Incapacitated Adults causing bodily injury, one count of Abuse and Neglect of Incapacitated Adults causing serious bodily injury, and four counts of crimes against the elderly," according to Tupper.

After receiving a report on Wheeler's suspension regarding suspected physical abuse, Tupper opened an investigation.

The investigation is still ongoing at this time.

The victims range from ages 52 to 92.
